Notice for New Donors

Participation with LeukoLab requires the following:

• Valid phone number and email
• Photo ID
• Reliable transportation
• A valid social security number for tax purposes

The following is highly recommended:

• Residence within an hour of our facilities
• Some weekday morning availability

Please do not complete questionnaire at this time if you currently:

• Are pregnant or breastfeeding
• Are under the age of 18 years old
• Are under our weight requirement of 130 lbs (weight will be assessed on-site)
• Have donated blood in the past 8 weeks or are donating blood elsewhere

This questionnaire is to be completed only once. If you are already a qualified donor with us, please visit I Want To Donate Again. The information obtained from this screening will be kept confidential to the extent required by law. All information will be password protected to maintain confidentiality.
